About The Position

The Community Hospice has a wonderful opportunity for a per diem RN to work as part of our team caring for patients and families in local nursing homes in the capital region (Albany, Schenectady, Troy, etc). Availability needed for this per diem position would be during the week (Mon-Fri). Hours generally needed are 8a-4:30p. No weekends needed at this time. As an RN you will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families. Per diem needs would be days would be during the week (Monday to Friday) and hours would be 8 am to 4:30 pm. Requirements

• Current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing
• Ability to communicate effectively, verbally and written
• 1 year experience as an RN required
• Valid Driver's License
• Reliable vehicle
• High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

• BSN preferred
• Experience preferably in long term care
• Previous Hospice experience preferred but not required
