About The Position

This role involves providing ongoing assessments and monitoring, including vital signs, for residents with chronic health conditions. The Registered Nurse will maintain a registry of residents with chronic diseases, update Individualized Plans of Care (IPOCs), and provide disease-specific education to promote self-management. Responsibilities include reviewing chronic care diagnosis reports, scheduling chronic care visits, managing medications, and providing adherence counseling, patient goal setting, and behavior-change counseling. The nurse will also identify needs for new treatment regimens, conduct comprehensive wound assessments (examination, measurements, drainage, monitoring progression/healing), perform wound treatments (cleansing, debridement, dressing application), and create/manage individualized wound care treatment plans.

Requirements

  • Knowledge of professional nursing principles and techniques used in the assessment, care and treatment of individuals with mental illness, aging individuals, women, men, and culturally diverse individuals' unique healthcare needs respectively.
  • Knowledge of laws and regulations covering nursing practices relative to rules on resident/client protection for residents/clients with mental/psychological illness/disabilities.
  • Knowledge of principles of effective, verbal, written and group communication to accurately and effectively communicate job-related information.
  • Knowledge of medical terminology used in the assessment, care and treatment of residents/clients with mental/psychological illness/disabilities in order to support and assess mental health problems, work with physicians to develop care plans, assess crisis, provide support and education, and integrate internal and external mental health services.
  • Skills to complete specific training to perform in a distinct, complex and secure environment.

Nice To Haves

  • Experience in a psychiatric, health care facility, acute care, long-term care, or correction setting is preferred.

Responsibilities

  • Provide ongoing assessments and monitoring, obtaining and monitoring of vital signs.
  • Education and care coordination for residents with chronic health conditions.
  • Maintain a current registry of all residents diagnosed with a chronic disease(s).
  • Adding/updating IPOC's.
  • Provide disease specific education and promote self-management of chronic health conditions.
  • Routinely review chronic care diagnosis reports to verify all residents are identified and being evaluated.
  • Schedule initial and follow up chronic care visits.
  • Medication management and reconciliation.
  • Adherence counseling, patient goal setting and behavior-change counseling.
  • Provide care utilizing provider directed delegated orders.
  • Identify needs to establish new treatment regimens.
  • Conduct wound assessments to include examination, measurements, drainage, and monitoring for wound progression or healing.
  • Perform wound treatment and procedures to include cleansing of the wound, debridement, and application of dressings.
  • Create, manage and update individualized wound care treatment plans.
