About The Position
Affirmed Home Care, New Jersey's premier private-pay home care agency, is seeking an experienced and compassionate Per Diem Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight throughout Bergen, Essex, Morris, and Passaic Counties. This field-based position is ideal for an independent, organized RN who enjoys building meaningful relationships with clients while delivering high-quality, personalized care in the comfort of their homes.
Requirements
- Active New Jersey Registered Nurse (RN) License.
- Minimum of 2 years of recent RN experience in acute care, home care, or case management.
- Recent experience with: Ventilator and tracheostomy care, G-tube/PEG tube management, IV infusions, Wound care
- Current BLS Certification.
- Current physical examination (within the past year).
- Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable.
- MMR immunization documentation.
- Valid driver's license and reliable transportation.
- Authorized to work in the United States.
Responsibilities
- Conduct comprehensive in-home nursing assessments.
- Develop, implement, and oversee individualized plans of care.
- Obtain and maintain physician orders in compliance with New Jersey regulations.
- Monitor client conditions and promptly communicate significant changes.
- Complete accurate and timely clinical documentation.
- Perform aide supervisory visits and ensure quality of care.
- Provide clinical guidance and support to Home Health Aides and Nurses.
- Promote client safety, independence, comfort, and overall well-being.
