RN Case Manager for Hospice - Preceptor Home Health and Hospice - Fond Du Lac
About The Position
Preceptor Home Health & Hospice is seeking a full-time Registered Nurse (RN) Case Manager in Fond du Lac County, Wisconsin. The RN Case Manager oversees and coordinates patient care in collaboration with an interdisciplinary team, supporting patients and families and helping individuals receive care in their homes. The position pays up to $45 per hour and offers a $5,000 sign-on bonus. The role includes developing and updating individualized care plans, assessing patients, providing skilled nursing care, educating patients and caregivers, coordinating services, and supervising LPNs and Home Health Aides. Much of the documentation is completed in patients’ homes.
Requirements
- Current Registered Nurse (RN) license in the State of Wisconsin.
- Graduate of an accredited nursing program.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and organizational abilities.
- Ability to work independently and manage a caseload effectively.
- Valid driver’s license and a reliable, insured vehicle.
- Physical ability to perform patient care tasks, including lifting, standing, and mobility support.
Nice To Haves
- Home health or public health experience is preferred but not required.
Responsibilities
- Develop, implement, and update individualized plans of care.
- Perform comprehensive patient assessments and ongoing evaluations.
- Provide skilled nursing care and carry out physician-ordered treatments.
- Educate and counsel patients, families, and caregivers.
- Coordinate care with physicians and other healthcare professionals.
- Manage documentation, with a majority completed in the patient’s home.
- Lead and participate in case conferences and care planning discussions.
- Identify and implement discharge planning needs.
- Communicate patient status updates to physicians and care team members.
- Supervise and evaluate care provided by LPNs and Home Health Aides.
- Ensure necessary equipment and services are arranged for patients.
