Requirements
- Active Minnesota Registered Nurse (RN) license
- 3+ years of RN experience, home health preferred
- Strong clinical decision-making and assessment skills
- Confidence working independently in a field-based role
- Organized, accountable, and patient-focused
- Reliable transportation
Responsibilities
- Lead comprehensive nursing assessments and care planning
- Manage a home health caseload across homes and community settings
- Perform OASIS visits and coordinate interdisciplinary care
- Provide education, guidance, and clinical direction to patients and families
- Collaborate with physicians and care teams to drive outcomes
- Maintain accurate, compliant documentation
