Requirements

• Active Minnesota Registered Nurse (RN) license
• 3+ years of RN experience, home health preferred
• Strong assessment, care planning, and documentation skills
• Ability to work independently in a field-based role
• Reliable transportation and valid driver’s license

Responsibilities

• Complete comprehensive nursing assessments and care plans
• Perform OASIS visits and coordinate care across settings
• Provide skilled nursing care, education, and support to patients and families
• Collaborate with physicians, therapists, and interdisciplinary teams
• Manage a caseload of patients in homes, assisted living, and community settings
• Maintain accurate, timely documentation and regulatory compliance
