About The Position
We're seeking a compassionate Registered Nurse (RN) Case Manager to join our Home Health & Hospice team on the beautiful Central Oregon Coast. This role offers the opportunity to build meaningful relationships with patients and families, enjoy autonomy and flexibility while serving patients in their homes, and join a supportive interdisciplinary team dedicated to compassionate care. Signature Healthcare at Home provides professional, home-based medical care for patients needing rehabilitation and expert care following surgery, a hospital stay or other illness. They work with each patient’s primary care provider to develop a care plan which will meet their individual needs.
Requirements
- Current Oregon RN license
- Strong assessment and communication skills
- Current driver's license and reliable transportation
- Passion for patient-centered care

- Home Health or Hospice experience preferred
