About The Position

We’re looking for a dedicated Registered Nurse (RN) Case Manager who is passionate about delivering exceptional hospice care while building meaningful relationships with patients and families throughout the community. Requirements

  • A current, valid Florida Registered Nurse (RN) license.
  • A minimum of one year of inpatient nursing experience OR previous hospice, home health, or long-term care experience.
  • Excellent communication and relationship-building skills.
  • Strong clinical assessment, critical-thinking, and problem-solving abilities.
  • The ability to collaborate effectively with physicians, patients, families, caregivers, and members of the interdisciplinary team.
  • Strong organization and time-management skills with the ability to manage an assigned patient caseload.
  • A valid Florida driver’s license, reliable transportation, and current automobile insurance.
  • A passion for serving patients and families with compassion, dignity, and professionalism.

Responsibilities

  • Manage an assigned caseload of hospice patients and families.
  • Complete comprehensive patient assessments and ongoing evaluations.
  • Develop, implement, and update individualized plans of care.
  • Support effective pain and symptom management.
  • Coordinate care with physicians and members of the interdisciplinary team.
  • Provide compassionate education, guidance, and emotional support to patients and families.
  • Supervise Hospice Aides and Licensed Practical Nurses as appropriate.
  • Maintain accurate, complete, and timely electronic medical record documentation.
  • Communicate changes in patient condition and care needs to the appropriate clinical team members.
  • Participate in interdisciplinary meetings and quality improvement initiatives.
  • Provide patient-centered hospice care wherever patients call home.
  • Deliver exceptional end-of-life care while upholding Big Bend Hospice’s mission and values.
