Position Summary As an RN Case Manager, you will oversee the care of hospice patients throughout their end-of-life journey. You'll manage a patient caseload while collaborating with physicians, social workers, hospice aides, spiritual care, bereavement counselors, volunteers, and other members of the hospice team to ensure every patient receives compassionate, individualized care. Patients are cared for wherever they call home, including: • Private residences • Assisted living facilities • Skilled nursing facilities • Residential hospice • Other community settings

Requirements

  • Current New York State Registered Nurse (RN) license
  • Strong clinical skills in bedside care, symptom management, crisis intervention, and family intervention
  • Ability to work independently while collaborating with a multidisciplinary team
  • Excellent communication and organizational skills

Nice To Haves

  • Minimum of 2 years’ experience as a Registered Professional Nurse

Responsibilities

  • Develop and oversee individualized plans of care
  • Assess patients and proactively manage symptoms and pain
  • Educate and support patients and families through every stage of the hospice journey
  • Coordinate medications, durable medical equipment, and specialty services
  • Collaborate closely with physicians and the interdisciplinary team
  • Participate in Morning Report and Interdisciplinary Group (IDG) meetings
  • Provide compassionate crisis intervention and symptom management when needed
  • Advocate for patients while ensuring exceptional quality of care
