About The Position

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life. Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth. Requirements

• Active Kansas or Compact RN license
• Valid driver’s license and auto insurance

Nice To Haves

• Hospice experience preferred; training provided
• Hospice experience preferred — but we will train the right nurse

Responsibilities

• Provide compassionate, hands-on hospice care
• Support and educate patients and families
• Complete assessments and ongoing evaluations
• Collaborate with physicians and the hospice team
• Address patients’ physical, emotional, and comfort needs
