About The Position

This position is for a Registered Nurse (RN) Case Manager at Memorial Medical Center. The role involves assisting with discharge planning, ensuring patients have the necessary resources for a smooth transition from the hospital. Responsibilities include coordinating with third-party payers, government programs, and community services to arrange care, housing, and transportation. The RN will coordinate placement and support services such as long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME. Additionally, the role assists with CPS, APS, adoptions, and end-of-life planning, including advance directives. The position aims to make a direct impact on patient care by navigating complex discharge processes and fostering continuity of care.

Requirements

  • Current state RN license
  • Associate’s degree from an accredited nursing school
  • Current licensure applicable to professional affiliation required.

Nice To Haves

  • Certification by the National Association of Healthcare Professionals or InterQual is desirable.
  • Certification as CCM or ACM desired.
