The Registered Nurse (RN) Care Manager will facilitate communication among patients/families, multidisciplinary teams, medical management teams, and community resources to identify and resolve barriers to timely care delivery. This role involves understanding and interpreting contracts to achieve optimal clinical and financial outcomes. The nurse will collaborate with patients, families, and inter-professional teams to ensure quality, efficient, and cost-effective healthcare services, potentially working embedded in a provider office or telephonically. Utilizing evidence-based approaches, the RN will promote patient and family activation and engagement, partnering with them to set SMART goals and assisting in the development of self-management educational resources. The position requires identifying potential learning barriers and reporting abnormal findings to the care team for collaborative planning. The RN will independently manage their caseload, ensuring timely completion of tasks and documentation for MCO, regulatory, and contractual requirements. They will partner with at-risk patients throughout their care journey, anticipating needs, responding to status changes, and prioritizing care based on essential patient needs. The role also involves coordinating patient information and communication between various parties for smooth transitions of care and coordinating referrals to internal and external resources. The nurse must demonstrate age-specific knowledge and skills in providing patient care.
