The Registered Nurse (RN) Care Manager - Ambulatory position is a full-time role within the GCMG SHVI: Kenilworth - Cardiology department. The primary focus is to facilitate communication among patients, families, multidisciplinary teams, and other resources to identify and resolve barriers to care. This role involves interpreting contracts to achieve optimal clinical and financial outcomes, and collaborating with various care teams to ensure quality, efficient, and cost-effective healthcare services. The nurse will utilize evidence-based approaches to enhance patient and family engagement in their care, develop SMART goals, and assist in the development of self-management educational resources. They will identify barriers to learning and care delivery, report abnormal findings, and manage their caseload independently, ensuring timely completion of tasks and documentation related to MCO, regulatory, and contractual requirements. The RN will partner with at-risk patients throughout their care journey to ensure continuity, anticipate needs, and respond to changes in patient status. Coordination of patient information and communication between various entities, including referring facilities, physicians, and community caregivers, is crucial for smooth transitions of care. Referrals to internal departments and external community resources will also be coordinated. The role requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient needs based on age-specific requirements.
