The Registered Nurse (RN) Care Manager - Ambulatory position focuses on facilitating communication among patients, families, multidisciplinary teams, and community resources to identify and resolve barriers to care. This role involves interpreting contracts to achieve optimal clinical and financial outcomes, and collaborating with various care teams to ensure quality, efficient, and cost-effective healthcare services. The position may be embedded within a provider office or conducted telephonically. The RN Care Manager utilizes evidence-based approaches to enhance patient and family activation and engagement, developing SMART goals with patients and families as appropriate. They also assist in developing patient self-management educational resources, identify barriers to learning or care delivery, and report abnormal findings to the responsible provider. The role requires independent caseload management, timely completion of documentation related to MCO, regulatory, and contractual requirements, and partnering with at-risk patients throughout their care journey to ensure continuity. The position also involves anticipating patient needs, responding to changes in status, and prioritizing care based on essential needs. Coordination of patient information and communication between various entities, including referring facilities, physicians, and community caregivers, is crucial for smooth transitions of care. Referrals to internal departments and external community resources are also coordinated as necessary. The role requires demonstrating knowledge and skills appropriate for the age of patients served, including understanding growth and development principles and assessing patient needs based on age-specific requirements.
