About The Position

Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. This role involves implementing and monitoring patient care plans, coordinating with various disciplines for patient care, and assessing discharge planning needs. The Registered Nurse (RN) is responsible for systematic data collection, problem identification, planning and outcomes, implementation of care, documentation, evaluation of patient status and care effectiveness, and care coordination. This includes actions initiated with patients, families, and healthcare team members to manage and correct the sequence, timing, and/or effectiveness of patient care from admission to discharge. The role also involves delegation, supervision, and prioritization of tasks.

Requirements

  • Graduation from an accredited School of Nursing.
  • Continuing education requirements as defined within the CARE program.
  • One (1) year as an RN.
  • Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days.
  • Basic Life Support (BLS) American Heart Association.

Responsibilities

  • Implements and monitors patient care plans.
  • Monitors, records and communicates patient condition as appropriate.
  • Serves as a primary coordinator of all disciplines for well-coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Performs initial assessment, re-assessment, and focused assessment based on patient condition, setting and population.
  • Identifies patient problems based on clinical judgment of responses to health conditions or needs.
  • Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
  • Formulates a plan of care, including transition or discharge planning.
  • Implements nursing care according to the care plan, ensuring continuity of care during hospitalization and preparation for discharge.
  • Documents care in the patient's record.
  • Ensures patient safety.
  • Administers medications.
  • Provides ordered treatments.
  • Educates patients and families.
  • Provides nursing care.
  • Documents in the healthcare record.
  • Continuously evaluates patient status and effectiveness of nursing care, modifying the care plan as needed.
  • Manages and corrects the sequence, timing, and/or effectiveness of patient care from admission to discharge through actions with patients, families, and healthcare team members.
