About The Position

We are seeking a dedicated Assessment Nurse (RN) to join our senior living community supporting Independent Living, Assisted Living, and Memory Care residents. In this role, you will be responsible for completing resident assessments, identifying changes in condition, and collaborating with the care team to ensure high-quality, individualized care. Requirements

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university.
  • Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care community, or other related health care community.
  • Must have, as a minimum six (6) months experience in rehabilitative and restorative nursing practices.
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state.
  • Must be able to proficiently operate a computer.
  • Must possess basic computer skills in Word, Excel, Outlook.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Responsibilities

  • Develop and maintain a good rapport with all parties involved with the service plan to ensure that a team effort is achieved in developing a comprehensive service plan.
  • Coordinate services amongst the Interdisciplinary Team.
  • Ensure that all personnel involved in providing care to the resident are aware of the resident’s service plan.
  • Ensure that nursing personnel refer to the resident’s service plan prior to administering daily care to the resident.
  • Review all physician orders timely.
  • Complete Change in condition assessments per company policy.
  • Complete Quarterly assessments per company policy.
  • Complete admission assessments required by Registered Nurse per company policy.
  • Participate in developing the Service Plan when residents receive health services.
  • Make recommendations for service plans as needed.
  • Assist with the assessment and selection of residents for admission to the community, if applicable
  • Work well with others and promote teamwork.
  • Assure residents’ rights are known and followed.
  • All other duties as assigned.
