The Registered Nurse (RN) – Assertive Community Treatment (ACT) Team provides comprehensive nursing care and clinical support to individuals with serious and persistent mental illness and co-occurring disorders in a community-based setting. Working as part of a multidisciplinary ACT Team, the RN is responsible for medication management, health assessments, care coordination, health education, crisis intervention, and treatment planning. The RN plays a critical role in supporting recovery-oriented services that promote wellness, stability, independence, and successful community living. This position provides care in homes, community settings, and other locations where clients live, work, and receive services.
