About The Position
Matrix Home Care is currently seeking experienced and compassionate Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide Start of Care (SOC) assessments and skilled nursing visits throughout Brevard County, Florida. We are looking for clinicians who are passionate about patient-centered care and interested in joining a supportive and growing home health organization.
Requirements
- Active Florida RN license (for RN position)
- Active Florida LPN license (for LPN position)
- Valid driver’s license, reliable transportation, and auto insurance required
- Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
Nice To Haves
- Home health experience preferred (for RN)
- OASIS documentation experience strongly preferred (for RN)
- KanTime EMR experience preferred (for RN and LPN)
- Home health and skilled nursing visit experience preferred (for LPN)
Responsibilities
- Complete Start of Care (SOC), recertification, resumption of care, and routine skilled nursing visits
- Perform comprehensive patient assessments and documentation
- Develop and implement individualized care plans
- Educate patients and families regarding medications, treatments, and disease management
- Maintain accurate and timely Medicare documentation
- Collaborate with physicians and interdisciplinary healthcare teams
- Ensure compliance with company policies and home health regulations
