About The Position
Concierge Home Care is seeking a Home Health Registered Nurse (RN) for after-hours weekday coverage in Jacksonville/Duval County, FL. This part-time/modified full-time role offers a flexible schedule with evening-focused shifts, providing greater work-life balance and an independent patient care environment. The position eliminates hospital shifts and 12-hour rotations. The role utilizes technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, allowing nurses to focus more on patient care. Requirements
- Active Florida RN license or Compact RN license
- Strong assessment and triage skills
- Ability to work independently
- Reliable transportation, valid driver's license, and auto insurance

Responsibilities
- Provide clinical support outside of traditional business hours to ensure continuity of care and reduce hospitalizations.
- Complete skilled nursing visits during weekday evenings.
- Perform Start of Care (SOC) assessments, Resumption of Care visits, PRN visits, and urgent nursing visits beginning at 3-4:00 PM.
- Respond to after-hours patient calls and triage concerns appropriately.
- Coordinate care with physicians, case managers, and interdisciplinary care teams.
- Provide skilled clinical care including comprehensive patient assessments, medication reconciliation, wound care, disease management education, and catheter care.
- Support urology-related patient needs including Foley catheter management, nephrostomy care, urinary retention monitoring, post-surgical urology follow-up, and infection prevention education.
- Complete documentation within required EMR timeframes.
