Requirements

Currently licensed as a Registered Nurse (RN) in the state of Idaho
Demonstrate knowledge and skill in nursing practice
CPR professional certification
General knowledge of computers and the ability to learn industry-specific software and documentation applications
Possess a valid state driver’s license and automobile liability insurance

Nice To Haves

A minimum of one year of clinical experience is preferred but not required

Responsibilities

Work with patients, their families, and other healthcare professionals in the patient’s place of residence.
Perform visits and complete coordination of care.
Serve as the point of contact for all disciplines involved with providing care to the patients.
Work with fellow clinicians to train and educate them regarding best practices.
Assist the Team Lead and/or Director in completing the tasks assigned to the Department.
Follow agency, state, and federal policies and procedures.
Consult with the patient’s primary care physician.
Collect OASIS information (Home Health only).
Ensure medication profiles are accurate.
Attend meetings as required.
