Requirements
- Currently licensed as a Registered Nurse (RN) in the state of Idaho.
- CPR professional certification.
- General knowledge of computers and the ability to learn industry-specific software and documentation applications.
- Possess a valid state driver’s license and automobile liability insurance.
Nice To Haves
- A minimum of one year of clinical experience is preferred but not required.
Responsibilities
- Work with patients, their families, and other healthcare professionals in the patient’s place of residence.
- Perform visits and complete coordination of care.
- Serve as the point of contact for all disciplines involved in patient care.
- Train and educate fellow clinicians on best practices.
- Assist the Team Lead and/or Director in completing departmental tasks.
- Follow agency, state, and federal policies and procedures.
- Consult with the patient’s primary care physician.
- Collect OASIS information (Home Health only).
- Ensure medication profiles are accurate.
- Attend meetings as required.
