About The Position

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.

Requirements

Graduate from an accredited School of Nursing.
Current license to practice as a Registered Professional Nurse in New York State.
BCLS, ACLS, required.
2+ years Telemetry experience, required.

Bachelor’s Degree in Nursing, preferred.
Must be enrolled in an accredited program within 24 months of employment, if hired after January 1, 2018 and obtain a BSN Degree within five (5) years of employment date.

Responsibilities

Assesses the patient’s physiologic health status, including gastrointestinal, cardiovascular, renal, neurological, and skin condition, and adjusts the plan of care accordingly. Assesses level of pain and pain management, communicates and documents patient’s physiologic health status and plan of care.
Assesses patient’s psychosocial health status, including perception of medical/nursing care, coping mechanisms, knowledge level, cultural/ethnic requirements, and provides emotional support. Communicates and documents psychological status and care plan.
Identifies patient outcomes by developing criteria for measurement, identifying actual/potential patient problems, and assessing patient's need for teaching based on psychosocial and developmental assessment. Develops patient outcome statements and establishes individualized patient goals.
Formulates the plan of care and establishes priorities necessary to achieve expected outcomes, coordinating the cost-effective use of supplies, equipment, and medication. Documents the plan of care and collaborates with physicians and other health team members.
Implements the plan of care by maintaining vigilance over patient safety, exercising professional skills, reassessing patients as needed, correctly administering treatments and medications, using equipment correctly, providing emotional support, applying scientific principles safely and efficiently, making accurate observations, keeping accurate documentation, acting rapidly and effectively in emergencies, providing a calm atmosphere, communicating effectively with family/caretakers, participating in discharge planning, and providing discharge instructions.
Evaluates care provided for patient outcomes by demonstrating ability to measure effectiveness, performing variance analysis, designing/implementing/evaluating systems to improve care, and keeping accurate documentation.
Collaborates with other care team members in planning and carrying out treatment regimens, providing direction to LPNs, Patient Care Technicians, Support Care Associates, and Nursing Unit Secretaries. Collaborates with physicians, interprets and implements treatment regimens, assists physicians during procedures, keeps management/physicians informed of patient changes, and uses clinical judgment in delegating assignments.
Performs grade I-IV Pressure Injury Care, including preventive skin care, applying simple dressings, warm & cold compresses, and irrigations, consistent with facility procedure.
Administers medications correctly and safely by identifying medication action, dosage, side effects, and implications for the patient. Meets standard on medication administration examination, demonstrates preparation of local solutions and I.V. drips, administers and documents medication correctly, administers IVP medication and IV standard infusions per department policy, and educates patients and significant others related to drug and food interactions.
Participates in patient and family education by explaining the patient's condition, communicating assessment data, recognizing and utilizing teaching opportunities and resources, providing early discharge planning and appropriate referrals, and evaluating the effectiveness of teaching.
