About The Position

This is a grant funded opportunity for a Registered Nurse. As the Program Care Team Nurse, you will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity of patient care, and report patient progress. This position will work closely with the medical staff.

Requirements

  • Registered Nurse licensed in the State of Indiana
  • Current CPR Certification
  • Strong organizational and time management skills
  • Proven ability to work well in a team environment
  • Excellent written and verbal communication skills
  • Ability to remain flexible and adaptable
  • Ability to follow HealthLinc policies and procedures
  • Operate a multi-line phone system and other office equipment including printers, fax machines, etc.
  • Basic software skills (Microsoft Office, EHR, online sources, etc.)

Nice To Haves

  • Bachelor of Science in Nursing
  • At least 1-year ambulatory or public health nursing experience

Responsibilities

  • Review dashboard to monitor patient vitals.
  • Performs assessment, data collection, obtain, review and analyze information in collaboration with the healthcare team members and others as appropriate.
  • Identifies unsafe patient environment and intervenes responsibly.
  • Discusses situations with Medical Providers, Pediatricians and Nurse Practitioners for development of emergency plan for patient and family regarding their health care.
  • Assist with management of patient calls with concerns of their vitals or utilization of device.
  • Assist with scheduling on site appointments with patient care team nurse or Pharmacist.
  • Assists patients and families in functioning at their optimum level.
  • Collaborate with the Program Director on any monitoring and tracking of devices
  • Participates in continuous quality improvement activities by evaluating patient care systems that may include standards, protocols, and documentation.
  • Collaborates with the healthcare team to ensure continuity of patient care throughout the healthcare settings.
  • Promotes effective communication among healthcare team members including patient, family and significant others.
  • Utilize EHR to document patient contact.
  • Keeps care coordination records up to date.
  • Initiates, implements, and documents patient and family teaching and documents their response to teaching.
  • Assists in implementing and carrying out continuous quality improvement programs and activities.
  • Coordinates services for patients and families utilizing other health related agencies and resources.
  • Obtains referrals to other physicians and specialists.
  • Participates in program meetings.
  • Follows approved protocols/standing orders for patient care.
  • Ensures that a productive and efficient patient flow occurs.
