About The Position

James River Home Health is actively seeking a compassionate and dedicated PRN Home Health RN Case Manager to support our patients in the Prince William territory. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.

Requirements

Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available during work hours.

Nice To Haves

At least one year of home health experience preferred
OASIS and HCHB experience PLUS!

Responsibilities

Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition.
Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team.
Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.
