About The Position

This Registered Nurse position in Primary Care at Lebanon, TN, is responsible for applying the nursing process to plan and implement patient care, including patient and family education and continuum of care planning. The nurse will effectively communicate pertinent patient/family information to the health care team and collaborate in establishing patient goals and evaluating progress to ensure effective outcomes. This role furthers professional growth by engaging in learning activities according to the Board of Nursing standards. The nurse will analyze comprehensive information relevant to patient care and management to determine nursing diagnoses and implement evidence-based interventions with triage protocols. This position involves telephone communication, requiring time management, prioritization, flexibility, and autonomy. The nurse will work with several providers and other healthcare roles, maintaining clinic skills for onsite performance, including patient intake, education, nurse visits, injections, vaccine and medication administration, service recovery, clinical primary care services, and EKGs.

Requirements

  • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • Graduate of an approved discipline specific program
  • 1 year of experience

Responsibilities

  • Applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning.

  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
