About The Position

Guide seamless care transitions by serving as the primary liaison between patients, physicians, leadership, and community partners to ensure aligned, compassionate patient care. Elevate standards of care through active coordination of clinical treatment plans, quality improvement initiatives, and educational programming across the department. Drive process improvements by tracking key patient outcomes and analyzing performance data to optimize operational efficiency and healthcare delivery. Bridge healthcare access gaps by connecting uninsured and underinsured patients with vital community resources and follow-up care pathways.

Requirements

Registered Nurse credentialed from the Texas Board of Nursing required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Extensive background in case management.
Strong computer proficiency.
Adaptability to navigate flexible work requirements.
Hands-on experience conducting community and home visits (reliable personal transportation required).

BLS Provider preferred.
Bilingual candidates.
Pediatric care experience.
