About The Position

Spectrum Health Care is seeking full and part-time Registered Nurses (RNs) to join a specialized team delivering an integrated, one-team, community palliative care model for the Etobicoke community. This role involves working collaboratively with health system partners to support patients and their families throughout their palliative care journey. The position offers a unique opportunity to be part of a tight-knit, collaborative team from across the health sector, delivering the next generation of palliative care in Etobicoke. The role focuses on providing holistic, proactive, seamless care and support to patients, families, and caregivers, aiming to enhance quality of life, comfort, dignity, and security during the end-of-life journey. As an integral member of the interdisciplinary care team, the Nurse, Palliative Care provides holistic, patient-centered care, including symptom management, care coordination, patient/family/caregiver education, emotional support, and advocacy for assigned patients or caseloads.

Requirements

  • Currently registered and in good standing with the College of Nurses of Ontario
  • Minimum of two-years nursing experience
  • Must have experience working with palliative care patients
  • Current police check suitable for working with vulnerable sector
  • Valid Ontario driver's license and reliable vehicle with willingness to travel
  • Current CPR and First Aid Certificate.
  • Demonstrated competence related to a variety of nursing skills including, but not limited to, wound care, IV therapy, chronic disease management, and palliative care and pain management

Nice To Haves

  • 2 years of palliative care experience preferred
  • Successful completion of any of the following is considered an asset: Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or Fundamentals of Palliative Care; The Comprehensive Advanced Palliative Care Education (CAPCE)

Responsibilities

  • Works under the supervision of the Integrated Care Program Supervisor.
  • Collaborates with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of care.
  • Participates in clinical huddles and case conferences with team members such as physicians and allied health.
  • Practices in accordance with the College of Nurses of Ontario (CNO), the Regulated Health Professions Act and Spectrum Health Care standards, policies and procedures.
  • Travels within assigned region between patients’ residences or facilities.
