About The Position

Mercy Home Health, a member of Trinity Health at Home, is seeking a dedicated Full-time Registered Nurse for our Palliative Care program. This position is based out of South Broad Street and supports North, South, and West Philadelphia. A $10K Sign-on bonus is offered. Current employees and prior employees who have left within a year are not eligible for Sign-on or referral bonuses. Requirements

• BSN required.
• Current PA RN License and CPR Certification.
• Hospice/Palliative Care Certification required.
• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• Palliative Care Certification is required.
• Must have a current driver’s license and reliable transportation.

Nice To Haves

• Home care experience.
• Strong clinical knowledge base.
• Hospice experience preferred.
• One (1) year of experience as a professional care nurse in home care or hospice care is a plus.

Responsibilities

• Provide comprehensive care to patients and families in the palliative care program.
• Offer clinical consultation in palliative care.
• Develop, coordinate, and evaluate palliative and end-of-life care programs.
