As a Palliative Care RN, you'll play a meaningful role in supporting patients and families facing serious illness. Current Registered Nurse (RN) license in the state of practice. Nurses working under a compact license must transfer their license to the state of practice within 60 days of employment.
Bachelor of Science in Nursing (BSN) from an accredited institution. Education will be verified.
Basic Life Support (BLS) certification for Healthcare Providers.
RNs with less than 12 months of experience must obtain a Bachelor of Science in Nursing (BSN) within four (4) years of hire.

Nice To Haves

Experience in palliative care, hospice, oncology, primary care, care management, case management, critical care, or medical-surgical nursing.
Experience supporting patients with chronic or serious illness and complex symptom management.
Experience with advance care planning, goals-of-care discussions, and care coordination preferred.

Responsibilities

Coordinate care and serve as a navigator for palliative care patients and their families.
Provide patient and family education regarding diagnosis, treatment options, and available resources.
Assess symptoms and collaborate with providers to support symptom management plans.
Triage patient and family calls, questions, and care concerns.
Coordinate services across primary care, specialty care, hospice, home health, and community resources.
Support advance care planning conversations and documentation.
Provide ongoing follow-up and care coordination throughout the patient's healthcare journey.
