About The Position

Park Nicollet is looking to hire a Registered Nurse to join our Hospice team! Position Summary: The Outpatient Hospice Case Manager registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. This position will support the West Team (ex: Lake Minnetonka area)

Requirements

  • Baccalaureate degree in Nursing required.
  • Work experience as a registered nurse for at least two years.
  • Current license to practice as a professional nurse in Minnesota.
  • Must be able to function independently with minimal supervision.
  • High level of sensitivity, assessment skills, technical-skills, and communications skills.
  • Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.

Nice To Haves

  • Experience in hospice desirable.
