About The Position

The Forensic Assertive Community Treatment (FACT) program is an evidence-based, interdisciplinary team model serving individuals with severe mental illness, often with co-occurring substance use disorders, many of whom are involved in the forensic system. FACT provides intensive, community-based services focused on promoting symptom stability, improving coping and social skills, and supporting independent living and community integration. Services include wraparound care coordination, medication management, psychoeducation, substance use counseling, peer support, vocational services, and coordination with community and justice-system partners. As a Registered Nurse – FACT, you will be an essential member of this multidisciplinary team, providing person-centered nursing care, medication support, and health monitoring that improves outcomes for individuals receiving services in the community.

Requirements

  • Current, active Colorado Registered Nurse (RN) license
  • Minimum of 2 years of clinical nursing experience, preferably in behavioral health or a related setting
  • Demonstrated ability in nursing assessment including evaluation and interpretation of medical and behavioral health information.
  • Knowledge and familiarity of nursing care management concepts, and psychotropic medications.
  • Possess advanced written and oral communication, decision making and problem-solving skills, efficient time management with multi-tasking ability.
  • Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams.
  • Ability to operate PC-based software programs including proficiency in Word, Excel and other computer-based systems

Responsibilities

  • Provide routine medication delivery and support to clients, caregivers/family members in the community, including administering long-acting injectable antipsychotics.
  • Document clear and concise notes regarding patients’ condition, reaction to medication, and treatments in EHR.
  • Provide medical support to the psychiatrist.
  • When appropriate, blood samples are collected in the community for processing. When samples cannot be collected in the community, nursing will assist a client with transportation to the lab and/or coordination of care with outside agencies.
  • Monitor clients for signs and symptoms of acute illness and for changes in long-term chronic disease. This includes routine monitoring of vital signs and key health metrics, which will be entered into the EHR.
  • Instruct client/caregiver regarding medications and treatment instructions, tests ordered (i.e., lab orders).
  • Educate the client/caregiver on possible lifestyle changes that may be necessary to promote optimal health.
  • Completes Prior Authorizations as required.
  • Adheres to agency policy and procedure as well as clinic protocols.
  • Maintain a good working knowledge of community resources for basic needs such as food, shelter, and clothing when applicable.
  • Provide clients with psychosocial services to help support successful functioning in the community including ADLs, education, employment, family functioning, and recreation.
  • Provide assessment for high-risk clients and, when appropriate, follow agency protocols for ensuing client safety.
  • Be available to respond to crises per agency policy/protocol. This may include coordination with third parties and/or law enforcement.
  • Be flexible with schedule, which may include non-traditional hours, weekends, holidays and possible on-call.
  • Utilize program-issued cell phone during work hours for team-related issues. Staff are expected to be responsive to calls and electronic communication when scheduled to work.
  • Coordinate with community providers for continuity of care and advocate for clients.
  • Support the clinical functions of the program team (completing all medical orders and communicating vital patient care information to physician/prescriber.)
  • Perform nursing interventions within the scope of practice, congruent with licensing.
  • Perform other duties as required within the scope of the position and the employee's experience, education and ability.
