About The Position

Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response. Responds to/refers incoming patient treatment-related phone calls. Documents patient assessment and intervention data using electronic medical record and documentation practices. Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management. Maintains/reviews patient records, charts and other pertinent information. Posts tests and examination results. Delegates as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions. Use critical thinking skills within the principles of the nursing process and maintain appropriate written documentation to reflect the accuracy of care. To provide appropriate education to patients and families to promote health, facilitate rehabilitation and maintain wellness. To cooperate and collaborate with all members of the health care team. To apply ethical principles in caring for the patient and serving the family. To carry out duties and responsibilities per legal requirement under their Scope of Practice. Actively lead a culture of safe care per the Vision and Values of Community Memorial Hospital All other duties and responsibilities as designated by the Clinic and Physician Director.

Requirements

  • Associate degree, bachelor degrees from an approved NYS accredited nursing program
  • Current licensure by NYS
  • Certification in BLS, CPR, ACLS
