Requirements

  • Possess current skilled nursing facility/hospital experience (within 5 years) of being a new nursing graduate (within 2 years).
  • State of IL RN license that’s active and in good standing.
  • BLS certification.

Nice To Haves

  • At least 1 year of experience in a skilled nursing facility.

Responsibilities

  • Utilizes the nursing process to ensure assessments are performed in a timely manner and appropriate for patients' condition. Plan of care is formulated timely and addresses all the patients conditions and/or concerns. Interventions are timely and appropriate with reassessment and re-intervention occurring per policy and as indicated by residents condition.
  • Recognizes emergencies promptly and responds in an appropriate manner. Takes responsibility for crash cart readiness, dept readiness and is proficient in emergency procedures.
  • Administers medications and IV’s timely and accurately.
  • Performs treatments and procedures skillfully and on time as appropriate to the Resident Home.
  • Educates residents and families: assesses readiness to learn and barriers to learning. Delivers information on a level appropriate for the individual. Information is complete and documented.
  • Allows/encourages residents participation in decision making.
  • Applies knowledge of National Patient Safety Goals.
  • Assures that hourly rounding is done assessing toilet needs, pain, positioning & possessions within easy reach. Documents times on white board and follows with documentation in the medical records.
  • Demonstrates an awareness of fall prevention techniques: Identifies residents at risk, communicates that risk to other caregivers, reports all falls or assisted falls.
  • Engages in infection control practices: uses personal protective equipment appropriately, is diligent with hand washing , demonstrates knowledge of isolation procedures by implementing proper types of isolation.
  • Documentation is timely and accurately reflects resident condition, progress and events. Never uses banned abbreviations in documentation.
  • Resident hand off follows established procedures, reports are promptly given and received with an opportunity for questions. Utilizes SBAR communication method.
  • Practices within ethical, regulatory and legal guidelines and other standards and legislation applicable to position.
  • Demonstrates familiarity with the electronic documentation system and uses it effectively and efficiently.
  • Demonstrates ability to access policy and procedures by electronic use. Participates in PI program and adapts behavior to improve quality and service.
  • Treats residents/families as guests. Introduces self, is attentive to needs, acknowledges and assists people waiting for attention, immediately acts to resolve problems. Answers call lights promptly.
  • Respects privacy and confidentiality of residents. Knock before entering a room, pull curtains as resident desires, keeps resident information confidential.
  • Maintains professional appearance and demeanor: well groomed, observes dress code, communicates caring, refrains from discussing hospital business in the community.
  • Creates a positive learning environment for students and new team members. Demonstrates acceptance, tolerance and maintains a friendly, helpful attitude.
  • Attends at least ⅔ of department meetings, in service programs, continuing education classes, etc to remain up to date with changes in practice and Resident Home policies and procedures.
  • Demonstrates a willingness to learn and apply new knowledge. Holds self to high standards to deliver to the best of ability and is open to change.
