About The Position

This position involves administering and overseeing medications, monitoring clients' vital signs and health information, and maintaining accurate records. The role includes administering injectable medications, educating clients on side effects, and obtaining preauthorization for prescriptions. Responsibilities also encompass transcribing medical orders, obtaining medical information, administering pregnancy tests and drug screens, maintaining inventory of medicines and medical supplies, and scheduling outside medical appointments. The nurse will monitor medical equipment, participate in medication management clinics, and may be asked to perform other related tasks. The job requires occasional exposure to threat of bodily harm and routine exposure to blood, body fluids, or tissue. Reliable transportation is necessary for frequent travel between work sites. Staff are expected to practice principle-centered leadership, focus on customer service, and adhere to safety and Joint Commission standards.

Requirements

  • Up to eighteen months of education or training beyond the high school level in nursing or related field of study.
  • License for Practical Nursing in Commonwealth of Kentucky
  • Graduation of an Accredited program
  • Minimum 6-12 months nursing experience.
  • Reliable transportation for frequent travel between work sites.
  • COVID and Influenza vaccinations required.

Responsibilities

  • Administers and oversees self-administered and prescribed medications per established schedules as ordered.
  • Monitors clients' vital signs and other physical health information, maintains accurate records reflecting the data, including general information relating to treatment results.
  • Administers injectable medications, ensuring accurate prescribed dosage and timely delivery, educating clients in management of side effects, and obtaining preauthorization of prescriptions/medications from pharmacy, when needed.
  • Completes and maintains accurate documentation relating to provision of care, medications, other client-related services, and advocacy, including staff notes, case reviews, letters, reports, etc.
  • Maintains client medical record/charts relating to admissions and discharges.
  • Transcribes medical provider orders and obtains requested medical information pertaining to clients.
  • Administers pregnancy tests and drug screens on clients and documents results as needed.
  • Maintains accurate inventory counts of all medicines and medical supplies and ensures adequate replacements are ordered on a timely basis.
  • Schedules outside medical appointments for clients, as ordered by Medical provider.
  • Monitors medical equipment and testing per CLIA standards.
  • Participates on team with MD/APRN/RN to manage the flow of medication management clinics.
