About The Position

The licensed Registered Nurse (RN) plans and provides professional nursing services and standards of practice in accordance with level of experience and education, state board of nursing and established policies and procedures. The RN integrates the art, science, leadership and knowledge of the nursing clinical practice through relationship-centered, compassionate, ethical and respectful direct / indirect care. The On-Site Virtual Registered Nurse (RN) is a member of the Nursing Care Team and is responsible for collaboration, communication and facilitation of optimal provision of care through a computerized and/or digital platform supporting daily management of care. This role involves the assessment, planning, implementation, coordination, monitoring and evaluation of the patient's plan of care from admission to discharge, utilizing clinical knowledge, critical thinking skills and the principles of case management and adult education to coordinate, implement and support the plan of care. The RN ensures a seamless, effective, efficient transition of care across the continuum and is responsible for monitoring quality metrics specific to the department and ensuring complete and accurate documentation in the patient record. The role encompasses being a mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

  • Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.
  • Valid RN licensure authorized in the applicable state(s) of practice/employment.
  • Valid driver’s license where required by assignment.
  • Specialty credentialing and educational degree according to clinical nursing practice specialty area.
  • Minimum five (5) years of RN experience; health condition management, which must include adult condition management.
  • Electronic Medical Record (EMR) experience.

Responsibilities

  • Assess, plan, implement, coordinate, monitor and evaluate the patient's plan of care from admission to discharge.
  • Utilize clinical knowledge, critical thinking skills and the principles of case management and adult education to coordinate, implement and support the plan of care.
  • Ensure a seamless, effective, efficient transition of care across the continuum.
  • Monitor quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
  • Perform the roles of mentor, clinician, educator, consultant/collaborator, and navigator.
