This position provides ongoing care for patients, including assessment, planning, intervention, and evaluation, tailored to their age-specific and developmental needs. The role involves assessing educational needs and providing teaching and counseling to patients and their families, integrating health promotion and preventive care. The nurse will assume primary responsibility for caseloads in a specialty area, anticipating and recognizing changes in patient status, and determining care priorities. They will assess patient responses to services and teaching, updating care plans as needed, and communicating with physicians to confirm and update the plan of care, providing regular status updates. The position also requires maintaining accurate clinical and patient records, providing telephone consultation/triage, participating in community education and quality improvement activities, and assisting with staff orientation and education. The nurse is expected to follow agency policies and procedures and continuously advance their knowledge and clinical skills in the specialty area, including pursuing certification. A strong understanding of age-specific care principles and the ability to interpret patient data are essential.
