The role involves coordinating and directing patient teaching and collaborating with health team members. In collaboration with the interdisciplinary care team, the nurse conducts initial and ongoing patient assessments, analyzes data, creates a plan of care, implements treatment, and evaluates its effectiveness. Responsibilities include administering medications safely and serving as a point of contact for patient care coordination across hospital departments. The nurse also acts as a patient safety advocate by participating in ongoing quality improvement initiatives within the department and integrating the professional practice model into care delivery.
