About The Position

The Neurosciences Department is seeking a Registered Nurse Navigator to support our growing Traumatic Brain Injury (TBI) Program. Working closely with a TBI Physiatrist and multidisciplinary care team, this role serves as a key resource for patients and families, ensuring seamless care coordination throughout the recovery journey. The Nurse Navigator acts as a patient advocate, helping guide patients through complex treatment plans while supporting exceptional clinical outcomes and patient experiences.

Requirements

Requires an Associates of Nursing (ASN).
Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
Requires that the RN has graduated from a nationally accredited nursing program.
Requires Basic Life Support certification through the AHA.
Requires knowledge of personal computers using various types of word processing and database software.
Requires clinical work experience, medical terminology.
Requires understanding of medical record requirements, regulations and policies.

Nice To Haves

Bachelors of Nursing (BSN) preferred.
3-5 years of experience preferred.
Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.

Responsibilities

Coordinate patient care from referral through treatment, follow-up, and transition of care.
Serve as a primary point of contact for patients, families, and caregivers.
Educate patients and families regarding diagnoses, treatment plans, and available resources.
Collaborate with physicians, nurses, therapists, case managers, and other team members to ensure timely, coordinated care.
Identify barriers to care and connect patients with appropriate support services and community resources.
Track and monitor program metrics, patient outcomes, and satisfaction measures.
Assist with quality improvement initiatives and program development efforts.
Support communication with referring providers and community partners.
Perform other duties that contribute to the growth and success of the TBI Program.
