About The Position
This role is for a Registered Nurse specializing in the Minimum Data Set (MDS) at Alexian Village of Tennessee in Signal Mountain, TN. The position is within the Skilled Care department and is a full-time, day shift role. The nurse will be responsible for collecting and verifying patient data for care plans, third-party reporting, and medical chart accuracy. They will also maintain adherence to Medicare, Medicaid, and long-term care regulations, drive continuous CMS survey preparedness, and collaborate with interdisciplinary teams to train staff on MDS-coordinated, resident-centered care plans.
Requirements
- Registered Nurse obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Responsibilities
- Collect and verify patient data for care plans, third-party reporting, and medical chart accuracy.
- Maintain strict adherence to Medicare, Medicaid, and federal/state/local long-term care regulations.
- Drive continuous CMS survey preparedness, participating in pre-survey prep, live surveys, and plans of correction.
- Partner with interdisciplinary teams and train staff on developing MDS-coordinated, resident-centered care plans.
