About The Position
The Registered Nurse (RN) provides recovery-focused services related to the safe and appropriate administration of medical treatment (including medications) as prescribed by the physician. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. AgeWise is a community-based program serving older adults. AgeWise serves a maximum of 600 individuals, aged 59+. Agewise's Strengths-Based Case Management is a consumer-driven and highly collaborative program. Teams work with members to identify needs and build a treatment plan that is collaborative and comprehensive. The goal is to assist people living in the community and ensure members are linked to housing, medical care, psychiatric care and natural supports. Agewise's Institutional Case Management (ICM) serves members who are conserved and placed in a long-term care facility. The program provides members with support in obtaining and maintaining benefits, care coordination, discharge planning, and advocacy. Members are assessed to ensure that they are placed in the least restrictive environment possible.
Requirements
- Licensure as a Registered Nurse in the state of program operations
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use your personal vehicle to attend meetings, etc.
