About The Position

At Telecare’s AgeWise program, we provide community-based, strengths-based case management and institutional care coordination for older adults (aged 59+). As a Case Manager III, you will serve as a primary support advocate, helping individuals with mental illness and co-occurring disorders navigate complex systems, build comprehensive care plans, and live as independently as possible in their community. Strengths-Based Case Management: Partner closely with older adults to identify their needs and connect them directly to vital resources, including housing, medical care, psychiatric services, and natural support networks. Institutional Care Coordination: Provide critical care coordination, advocacy, benefit navigation, and comprehensive discharge planning for conserved members placed in long-term care facilities—always striving to transition them to the least restrictive environment possible. Collaborative Support: Work within a highly collaborative team environment, providing direct support that empowers individuals to recover their health, hopes, and dreams.

Requirements

  • Bachelor’s degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associate's degree may be substituted for two (2) year of experience.
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.
