About The Position

Community-Based Nurse (RN or LPN) – ACT Team in Indianapolis, IN. This role is within the Adult Community Services (ACT Program) and is a Full-Time position, Monday–Friday. Join Adult & Child Health as a Community-Based Nurse (RN or LPN) on our Assertive Community Treatment (ACT) Team, where you’ll provide care directly in the community to individuals who need it most. In this role, you’ll support adults with serious mental illness and co-occurring conditions, helping them maintain stability, improve health outcomes, and live more independently. This position offers a unique combination of clinical care, autonomy, and meaningful relationship-building—all within a collaborative, mission-driven environment. Requirements

  • Current RN or LPN license in Indiana (or ability to obtain)
  • Willingness to obtain licensure in additional states as needed
  • Active RN or LPN license
  • BCLS certification
  • Strong communication and interpersonal skills
  • Ability to work independently in community settings
  • Comfort working with individuals experiencing mental health and substance use challenges
  • Proficiency with electronic health records
  • Ability to manage multiple priorities in a fast-paced environment
  • Valid driver’s license and ability to travel locally
  • Commitment to compassionate, person-centered care

Nice To Haves

  • Experience in behavioral health, community health, or primary care preferred
  • ACLS preferred

Responsibilities

  • Perform nursing assessments in community and clinic-based settings
  • Monitor physical and behavioral health needs of clients
  • Collect medical histories, complete screenings, and record vital signs
  • Collaborate with ACT team members to develop and implement care plans
  • Coordinate medical appointments and follow-up services
  • Ensure continuity of care across multiple providers
  • Administer medications and therapeutic injections as needed
  • Monitor medication compliance and side effects
  • Coordinate with pharmacies and providers for medication access
  • Educate clients on medications, chronic conditions, and wellness
  • Support disease prevention and harm reduction strategies
  • Advocate for client needs within healthcare and community systems
  • Maintain accurate and timely documentation in the EHR
  • Ensure compliance with regulatory and organizational standards
