About The Position

This role is for a Registered Nurse in the Long Term Care department at Franciscan Place in Brookfield, WI. The position involves rotating shifts and is PRN (as needed). The nurse will implement and monitor patient care plans, monitor and communicate patient conditions, serve as a primary coordinator for patient care across disciplines, and carry out physician and nursing orders. Additionally, the nurse will ensure resident safety by identifying and addressing hazards and emergency situations, and assess and coordinate patient discharge planning needs with the healthcare team.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Responsibilities

Implement and monitor patient care plans.
Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assure resident safety by identifying safety hazards and emergency situations and initiate corrective action as appropriate.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
