About The Position

We are an established Home and Community-Based Services (HCS) provider committed to delivering exceptional care to individuals with intellectual and developmental disabilities. The Role We are seeking a highly organized and proactive [RN / LVN] to oversee the medical and healthcare needs of the individuals in our HCS program. In this role, you will be the clinical anchor for our care teams, ensuring that our clients receive top-tier health monitoring and that our direct care staff are properly trained and supported. You will travel locally to group homes, host homes, or day habilitation centers in and around the Angleton area to ensure total compliance with Texas Health and Human Services (HHS) standards.

Requirements

  • Current, active, and unencumbered RN or LVN license in the State of Texas.
  • Minimum of [X] year(s) of clinical nursing experience.
  • Excellent clinical documentation skills.
  • Strong grasp of nursing delegation rules.
  • Ability to work independently.
  • Valid Texas driver’s license.
  • Auto insurance.
  • Reliable transportation for local travel between client homes in the Angleton area.

Nice To Haves

  • Prior experience within the Texas HCS or TxHmL waiver programs.
  • Experience working with the IDD (Intellectual and Developmental Disabilities) population.

Responsibilities

  • Conduct initial, annual, and change-of-condition Comprehensive Nursing Assessments (CNA) to evaluate health status and needs.
  • Develop, implement, and update individualized nursing service plans based on assessment findings and physician orders.
  • Train and officially delegate nursing tasks to unlicensed direct care staff in strict accordance with the Texas Board of Nursing regulations.
  • Conduct regular visits to individuals in their homes or day programs to monitor their overall health, review medication administration records (MARs), and evaluate the effectiveness of delegated tasks.
  • Maintain precise, audit-ready clinical records, nursing notes, and incident reports to ensure 100% compliance with Texas HHS guidelines for the HCS waiver program.
  • Act as a liaison between the individuals, their families, physicians, pharmacists, and the interdisciplinary team (IDT) to coordinate seamless, holistic care.
