About The Position

Progressive Community Health Centers is seeking an experienced professional to serve as a Registered Nurse/Licensed Practical Nurse! This is a full-time position that does not require late nights or holidays. The RN/LPN works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The RN/LPN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The RN/LPN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN/LPN establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The RN/LPN considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The RN/LPN participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

Requirements

  • Graduate of an accredited nursing program required.
  • Currently licensed in the State of Wisconsin required.
  • Previous experience with patients of all age groups required.
  • All hired candidates must submit proof of vaccination prior to their orientation.

Nice To Haves

  • Experience within a community health center and with EPIC preferred.
  • Two years of general nursing in a clinic or hospital setting strongly preferred.

Responsibilities

  • Provides coordinated, patient- and family-centered nursing care by actively collaborating with the interprofessional team to meet care needs across settings.
  • Telephone and MyChart triage: According to the chief complaint, assess the severity of the patient's symptoms and guide the patient to the appropriate level of care (Scheduled visit, Same day slot when available, Walk-In/Urgent Care, or Emergency).
  • Address all medication refill requests by directly refilling when allowable per protocol, pending prescription with correct pharmacy information and sending to the Provider team, or advising patient that an appointment is needed for further refills.
  • Review and triage the lab/x-ray results received in the office and notify the physician or other healthcare provider of critical values.
  • Empowers patients, families, and care partners by establishing shared goals, supporting care coordination, and using evidence-based information to guide informed decision-making.
  • Call patients with abnormal results as directed by providers and report abnormal results to governing bodies as directed (such as required STI reporting and lead reporting).
  • Perform patient education and instruction regarding medication regime and documents in the medical records.
  • Seeks educational opportunities to increase clinical competence, personal and professional growth through in-house education material, conferences, and community resources.
  • Demonstrates compassion through therapeutic relationships, attentive assessment, and respectful engagement with patients and families throughout the continuum of care.
  • Demonstrate tact, courtesy and respect in all interactions with patients, visitors, and staff as well as outside agencies.
  • Promote a positive working environment that represents cooperation and collaboration between programs, services and co-workers.
  • Ensures high-quality care by systematically assessing patient data, applying evidence-based practice, prioritizing safety and fiscal responsibility, participating in continuous quality improvement, and maintaining professional development.
  • Based on established protocols, administers oral, subcutaneous, intra-dermal, intramuscular medications.
  • Perform nurse visits and documents encounters within 72 hours.
