About The Position

Riverside Community Care has an exciting opportunity for a Registered Nurse/Licensed Practical Nurse to join our Outpatient Somerville team! The RN/LPN nurse is an integral part of the outpatient interdisciplinary treatment team. The nurse is responsible for providing direct care and care coordination for persons served primarily receiving medicated assisted treatment (MAT) but also supporting the needs of the general client population. This includes all duties related to patient care, such as medical screenings and assessments, follows up on lab work and referrals or admissions, and consults to other team members.

Requirements

• Registered Nurse or Licensed Practical Nurse license
• Direct patient care
• Care coordination
• Medicated Assisted Treatment (MAT) experience
• Medical screenings and assessments
• Follow up on lab work and referrals or admissions
• Consultation to team members

Responsibilities

• Assisting with filling prescriber orders
• Administering medications (buprenorphine and Vivitrol)
• Monitoring vital signs
• Provide guidance, education, and support in persons served recovery to allow them to maintain their physical and emotional health and live safely in the community.
