About The Position

Accurately assesses all wound, ostomy, and continence patients. Documents all pertinent wound/ostomy/continence information in the medical record. This includes, but is not limited to, wound measurements, descriptions, photographs, current treatment regimen, and interventions in place. Accurately inputs charges for procedures in the medical record. Uses critical thinking skills and evidence-based practice in the evaluation and selection of patient care products. Demonstrates proficiency with the application of wound dressings Demonstrates proficiency with the evaluation of ostomies and ostomy appliances. Professionally communicates with and educates the patient care team. Professionally communicates with and educates the patient and their family or other caregivers. Collaborates with the patient care team and establishes a plan of care using evidenced based practice. This also includes discharge planning, education, and DME supplies. Serves as a consultant and nurse expert providing evidence-based guidance and instruction on complex wound, ostomy and continence issues to peers, hospital committees, other entities, and home health agencies. Seeks out or maintains certification. Actively participates in continuing education and stays abreast of best practices in the field.

Requirements

  • Bachelors Nursing (BSN) is required or an agreement to begin BSN within (1) year and complete BSN or equivalence in MSN work within 5 years of hire is required.
  • Graduation from an accredited school of nursing required.
  • Advancement to this level requires a minimum of 9,800 hours of RN experience including three of the last five in the current specialty area.
  • Current RN licensure by the Virginia Board of Nursing required.
  • BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
  • Wound Ostomy Continence Nurse (WOCN) certification is required within 2 years of hire.
  • Meets all requirements of a Clinical Nurse III.
  • Competent to provide care using the cognitive, physical, emotional and chronological stages of human growth and development.
  • Competent to provide care using the nursing process.
  • Must have a reliable means of phone communication with hospital.
  • Required to submit evidence of PI project and two case studies annually with performance evaluation.
  • Required to work a minimum of 832 hours annually at VH.
  • Must exceed standards on Corporate Values portion of performance evaluation annually.
