About The Position

Olympic Health and Recovery Services is seeking a dedicated and compassionate RN to join our interdisciplinary team! The Homeless Outreach Stabilization and Transition (HOST) Program Registered Nurse (RN) is a field based position responsible for providing trauma informed, low barrier nursing care to individuals experiencing homelessness. This role functions entirely in the community, meeting people where they are, including sanctioned and unsanctioned encampments, wooded areas, shelters, vehicles, and public spaces. The RN must be comfortable working outdoors in all weather conditions and across varied terrain while delivering high quality, equitable care. The RN provides wound care, chronic condition support, medication management, appointment scheduling, and referral coordination. This position supports individuals with complex behavioral health, physical health, and substance use needs, helping stabilize participants, reduce barriers, and promote engagement with medical, behavioral health, and social service systems. This role works closely with HOST behavioral health staff, medical providers, and community partners to ensure continuity of care and holistic support.

Requirements

  • Current Washington State Registered Nurse (RN) license.
  • Experience in community based care, behavioral health, or addiction medicine preferred.
  • Must be able to pass required background checks as required by OHRS contractual requirements.
  • Current Washington State Driver's License, proof of insurance, and satisfactory drivers record required.
  • Commitment to racial equity, trauma informed care, and harm reduction principles.
  • Ability to work independently in diverse and non-traditional settings.
  • Ability to provide nursing care in high stress, high demand environments while communicating effectively.
  • Competency in basic medical care, wound care, and monitoring chronic conditions.

  • Flexibility, adaptability, and creativity when working with individuals with complex needs.
  • Competency in treating co occurring medical and behavioral health concerns.
  • Ability to utilize principles of crisis intervention and de escalation while maintaining strong customer service skills and empathy.
  • Ability to work collaboratively as part of a team and respectfully manage differences, opposing perspectives, opinions, and interests.
  • Ability to remain flexible and adaptable to change.

Responsibilities

  • Conduct nursing assessments in community based settings.
  • Provide wound care, foot care, and basic medical treatment.
  • Monitor chronic medical conditions and support ongoing health maintenance.
  • Evaluate patient needs and escalate concerns to medical providers as appropriate.
  • Develop individualized, person centered treatment plans aligned with harm reduction principles.
  • Create stabilization plans with an emphasis on accessibility and continuity.
  • Coordinate with HOST behavioral health staff to ensure follow through and integrated support.
  • Provide medication management and education in the field.
  • Support adherence to prescribed medications, including psychotropic and MAT medications.
  • Coordinate with prescribing providers regarding medication concerns and follow up needs.
  • Provide wound and foot care.
  • Support individuals with obtaining STI, HIV, and Hepatitis C testing.
  • Support individuals with obtaining birth control services.
  • Distribute naloxone and provide overdose prevention training.
  • Manage acute and chronic medical conditions.
  • Coordinate referrals to medical, behavioral health, substance use, and social service providers.
  • Schedule and support attendance at medical and psychiatric appointments.
  • Assist participants in navigating healthcare systems and accessing services.
  • Deliver all services in person, in the field, including encampments, shelters, and remote outdoor locations
  • Navigate uneven terrain and varied environmental conditions.
  • Engage individuals who may be disconnected from traditional health systems
  • Promote and encourage entry into behavioral health and substance use treatment.
  • Work closely with therapists, case managers, medical staff, and community partners.
  • Participate in interdisciplinary team meetings, case consultations, and training.
  • Provide supportive crisis intervention aligned with RN scope.
  • Utilize de-escalation and trauma informed techniques.
  • Maintain accurate and timely documentation of nursing services, assessments, and participant progress.
  • Adhere to state and federal regulations, professional standards, and agency policies.
  • Document in accordance with licensing and third-party requirements.
  • Participate in quality improvement initiatives using evidence based practices.
  • Provide backup coverage to the medical department as needed.
  • Support other programs to ensure continuity and accessibility of care across the organization
