Registered Nurse - Hospice Per Diem
About The Position
The Hospice RN will be responsible for initial, comprehensive and ongoing assessments, initiating the plan of care and making revisions as the patient's needs change, counseling patient and families in meeting nursing needs, communicating with the physician regarding patient's needs and reporting any changes in the patient's condition, obtaining and receiving physician's orders as required, and counseling field staff when the need arises. This is a per diem position. You will be placed on our roster for your specified city coverage list. As cases becomes available, we will contact you to check your availability and confirm your interest in accepting the assignments.
Requirements
- Graduate of an accredited school of nursing.
- Current licensure in California.
- CPR certification.
- Valid driver's license.
- Current physical and TB test required.
- Must be able to pass a background check.
- Excellent observation skills.
- Excellent verbal and written communication skills.
- Problem-solving skills.
- Basic math skills.
- Nursing skills per competency checklist.
Nice To Haves
- Previous hospice experience.
- Previous home health experience.
- Previous OASIS experience.
Responsibilities
- Initiate and revise the plan of care based on patient's changing needs.
- Counsel patients and families to meet their nursing needs.
- Communicate with physicians regarding patient needs and report changes in patient condition.
- Obtain and receive physician's orders as required.
- Counsel field staff when the need arises.

- Hospice Rates: START OF CARE: $300.00, SUPERVISORY: $100.00, RECERT: $150.00, DEATH: $150.00, PRN: $150.00
