About The Position

Haven Hospice is seeking a passionate, dedicated Hospice Admissions RN to join our team in Palatka, FL. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. This role offers a supportive, fulfilling environment where your skills and empathy can shine. The position involves visiting patients in various settings (homes, hospitals, nursing facilities, etc.) who are referred for hospice care. The RN will explain hospice services, discuss goals and care with patients and families, answer questions, and obtain necessary consents and legal documents. Responsibilities include obtaining medical history, current medications, and symptoms, performing comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines, coordinating the start of services (nursing, aides, equipment, medications), collaborating with the interdisciplinary team, and maintaining accurate and timely documentation.

Requirements

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
